GENERAL
These printable forms are in PDF (Portable Document) format. To read and print them, you need the free Adobe Acrobat Reader (which is probably already installed in your system). If you do need to install it, click below.


NEW: Complete & Print Forms - You can complete most of the forms listed below right on your computer before you print. Simply click on the form and type in the appropriate information. Then print the completed form, sign and mail it to the Plan Office. Forms with this feature are indicated below with the "COMPLETE & PRINT " notation.

  • Performer Information Form - COMPLETE & PRINT The "Performer Information Form" (formerly the Master Data Card) is the primary source for enrollment information for each participant. It also includes your designation of beneficiary. Please refer to the instructions on the second page when completing this card.
  • Change of Address Card - COMPLETE & PRINT The change of address card may be downloaded, completed, signed, and faxed or mailed to the plan office.
  • Annual Summary of Earnings - This is a record of all session and residual earnings reported on your behalf to the Pension and Health Plans during the calendar year. It also reflects your total Pension Credits and accrued benefits under the Pension Plan.
QDRO FORMS
  • QDRO Procedure - If you are separating or divorcing, you and your attorney will want to review this procedure.
  • Sample QDRO - This sample will help an attorney create a QDRO which will be acceptable to the Plan. For an editable version of this form in Word, click here.
STARTING THE RETIREMENT PROCESS
Before completing any pension forms, you may wish to confirm your eligibility for pension with the Pension Plan Office.
  • Pension Application - COMPLETE & PRINT You need to complete this form in order to begin the retirement process. Upon receipt of the completed form by the Plan, an estimate of your pension will be calculated and additional forms will be mailed to you.
  • Performer Information Form - COMPLETE & PRINT When you retire, you need to complete a new Performer Information Form.
  • Instructions Concerning Submission of Proof of Age - No more exaggerating your youth. The Plan must have proof that you are old enough (as detailed in this form) in order to process your pension application.
COMPLETING THE RETIREMENT PROCESS
Before completing any pension forms, you may wish to confirm your eligibility for pension with the Pension Plan Office.
  • 50% Husband & Wife Pension Rejection - COMPLETE & PRINT If you are married, your spouse must agree in writing to any form of pension payment other than the Husband & Wife form of pension payment using this. This document must be notarized.
  • Pension Payment Election Form - COMPLETE & PRINT This form is the document on which you select the form of your pension payment as well as whether you are choosing the Partial Lump Sum option. Note: The designation of beneficiary on this form supercedes the one on the Performer Information Form.
  • Explanation of Pension Forms - Here is a concise explanation of each of the forms of payment which may be available to you.
  • Rollover Election Certification - This form is required for a direct rollover to an IRA.
  • Direct Rollover Instructions to the Trustee - COMPLETE & PRINT Use this form to specify where the direct rollover payment will be paid.
  • Partial Lump Sum Pension Distribution - COMPLETE & PRINT This form is required when electing a Partial Lump Sum Pension distribution.
  • Special Tax Notice Regarding Plan Payment - This document gives a thorough explanation of the tax effects of taking a Partial Lump Sum. Read this to help decide whether to rollover your Partial Lump Sum or not. You may want to discuss these issues with your tax advisor.
  • QDRO/Court Order Acknowledgments - This is a required form which provides a brief explanation of a Qualified Domestic Relations Order (QDRO) or similar court order. You must sign this form acknowledging that it is your responsibility to inform the Plans of any such QDRO or court order.
  • Information Concerning Other Pension Plan Benefits - COMPLETE & PRINT This form is required in order to make sure the Plan is complying with tax laws regarding maximum benefits.
  • IRS FORM W-4P - The IRS form W-4P helps you to calculate and communicate to the Plan your witholding amounts for federal taxes.
  • Pension Benefit Tax Withholding Election Form - COMPLETE & PRINT This form specifies your California withholding and completes your election for federal tax withholding from your pension payments.
  • Direct Deposit Authorization Form - COMPLETE & PRINT You will certainly want your pension payments automatically deposited to your checking account. This form will allow you to make that happen.
  • Retirement Declaration and Signature Form - The signature form is required from each prospective retiree.
  • Acknowledgements And Certifications Form - COMPLETE & PRINT This form is required in order to make sure there are no misinterpretations regarding your retirement. Four signatures are needed on this page.
