
SENIOR PERFORMERS HEALTH PLAN ELIGIBILITY

The Senior Performers Health Plan primarily provides coverage for pensioners who are age 65 or older and their eligible dependents. Occupational Disability pensioners (whose disability was the result of an injury incurred while working under a Screen Actors Guild contract) are eligible for Senior Performers health coverage at any age.
You are eligible for Senior Performers Plan I coverage if you are receiving a pension based on at least 20 Pension Credits. Pensioners who have at least 15, but less than 20 Pension Credits can obtain coverage under Senior Performers Plan I by paying a premium equal to 25% of the actual cost of the coverage. Please contact the Plan Office for information
regarding Senior Performers premium payment procedures.
Exception: Participants who had at least 10 Pension Credits as of December 31, 2001 and were at least age 55 or older as of December 31, 2002 will qualify for Senior Performers benefits with no premium once they reach age 65 and are receiving a pension.
Senior Performers Health Plan eligibility will commence on the date you meet the eligibility requirements and will continue on a yearly basis. For Occupational Disability pensioners, coverage begins when Medicare Disability coverage begins.
Senior Performers Health Plan coverage is secondary to Medicare for hospital and medical benefits. You must be enrolled in Medicare Parts A and B to be eligible for full Plan coverage. Your spouse must also be enrolled in Medicare Parts A and B if he or she is age 65 or older, or is eligible for Medicare due to disability. You and your spouse do not need to enroll in a Medicare Part D Prescription Drug Program to receive Senior Performers coverage. Please refer to pages 71 and 72 for additional information regarding Medicare.
Your Senior Performers eligibility may be replaced with Earned Active coverage if you meet the minimum Covered Earnings requirement for Earned Eligibility based on sessional earnings or if you meet the minimum Days of Employment requirement. You will receive Earned Active Plan I coverage. If you are not required to pay the Senior Performers premium, you will not be required to pay the Earned Health Plan premium. If you are required to pay the Senior Performers premium, your premium will be reduced to the Earned Plan I amount. Your Senior Performers
premium will also be reduced to the Earned Plan I amount if you regain Earned Inactive eligibility. Please refer to Medicare,
page 72 for a description of Earned Active and Earned Inactive eligibility.
Important: Pension Credits earned under the Alternative Eligibility Program do not count toward eligibility for Senior Performers coverage.
