ACCIDENTAL DEATH AND DISMEMBERMENT (AD&D) BENEFITS

Eligibility
To qualify for the AD&D benefits, you must have Earned Eligibility under Plan I at the time of your loss. When you lose Plan I Earned Eligibility, your AD&D insurance coverage ends. These benefits are not available if you have Plan II Earned Eligibility or are covered under the Senior Performers Health Plan or the Self-Pay Program.

Accidental Death Benefit
If you are killed in an accident, or die within 90 days as a result of an accident, your beneficiary will be paid an accidental death benefit of $10,000. This is in addition to the $10,000 life insurance benefit.

Dismemberment Benefits
If you suffer the loss of sight or limb because of an accident or, within 90 days after an accident, the following benefits will be paid to you:

For Loss Of Your AD&D Benefit Equals
1 hand, 1 foot or sight of 1 eye $5,000
Any two or more of the above $10,000

Exclusions
AD&D benefits are not payable for losses due to:

  • Committing or trying to commit a felony or other serious crime or an assault.
  • Diagnosis of or treatment for physical or mental illness.
  • Infection, unless it is caused by an external wound that can be seen and which was sustained in an accident.
  • Intentional or reckless self destruction or injury, including but not limited to, suicide, acute intoxication from alcohol and/or drug overdose.
  • War or insurrection.
