HOSPITAL AND MAJOR MEDICAL LIFETIME MAXIMUM

The maximum amount the Plan will pay for hospital and major medical benefits during your lifetime is $2,000,000. A separate $2,000,000 lifetime maximum applies to each member of your family. Once you or your dependents begin to use hospital and major medical benefits, up to $1,000 of the lifetime maximum will be reinstated automatically at the end of each calendar year.
