CONVERSION OPTIONS

Hospital and Medical (See Update)
A conversion policy for hospital and medical coverage is available to you and your qualified dependents upon termination of eligibility. No test of insurability is required.

The insurance carrier and the requirements for conversion are different for California and non-California participants.

California Participants: You are entitled to purchase a hospital and medical conversion policy upon termination of Earned Eligibility or you can enroll in the Self-Pay Program first. You will then have the right to purchase conversion health coverage at the end of your maximum term of Self-Pay coverage.

You must submit an application and payment to Blue Cross of California within 31 days of the date you lose coverage. For applications, call Blue Cross at 1-800-333-0912.

Non-California Participants: You are entitled to purchase a hospital and medical conversion policy upon the completion of 18 months of Self-Pay coverage.

You must submit an application and payment to Celtic Life within 31 days of the date you lose Self-Pay coverage. For applications, call Celtic Life at 1-800-365-2365.

For both California and non-California participants, dependent children who lose dependent status because they become too old for coverage or marry may also convert their hospital and medical coverage, as may your spouse or same-sex domestic partner, in the event of divorce, dissolution of the partnership or your death. Applications must be submitted within 31 days of the loss of coverage. Dependents of non-California participants must exhaust 36 months of Self-Pay coverage before purchasing a conversion policy if Earned Eligibility was lost due to a change in dependent status. They must exhaust 18 months of Self-Pay coverage if Earned Eligibility was lost due to a reduction in your Covered Earnings or Days of Employment.

Life Insurance
Alife insurance conversion policy is available to the participant only at the time Plan I Earned Eligibility terminates. You may convert $5,000 if you are losing Plan I Earned Eligibility and gaining Senior Performers Eligibility. Otherwise you may convert $10,000.

You must submit an application and payment to Metropolitan Life Insurance Company (MetLife) within 31 days of the date you lose coverage. For applications call MetLife at 1-800-638-5433.
