When you apply for coverage, you will first choose a Nursing Home Daily Maximum Benefit (DMB) amount. The Nursing Home DMB is the most the insurance may pay for covered services received in a licensed nursing home on any day.
Each Nursing Home DMB has a corresponding Home Health Care DMB. Up to 60% of the Nursing Home DMB may be applied to the costs of covered services received in the insured's home on any day.
In addition, each DMB has two corresponding Lifetime Maximum Benefits (LMB), of which you will choose one. The LMB is the total pool of money payable for covered long-term care services received while you are insured.

For example, if you were to choose Option 4, the benefits available to you, should you become eligible, would be up to $200 per day for nursing home benefits or up to $120 per day for home care benefits, and up to $730,000 over your lifetime for all covered services. The premium rates for these options are based on your issue age when you apply. So the younger you are, the lower your rates will be.
The Long-Term Care Insurance Plan offered to participants in the Screen Actors Guild - Producers Health Plan includes all of these provisions and more. Modified Underwriting: During the initial enrollment period, May 1 - June 30, 2000, participants with Earned or Retiree eligibility may be able to qualify with simplified proof of good health by answering only two medical questions. Benefit Bank Option: If selected*, this option allows the insured to stop paying premiums after 3 years or more of premium payment and retain a portion of benefits.
Return of Premium at Death Provision: In the event of the insured's death before age 70, this provision allows a portion of premiums paid to be refunded to the insured's estate**.
Portability: Insureds who are no longer participants in the Health Plan can continue their coverage at the same group rates applicable to Plan participants.

Long-Term Care Insurance is not just coverage for nursing homes. In fact, many of the features of the John Hancock plan are designed to give the insured several choices as to how their long-term care is administered.
