How can I make a contribution payment for a choreographer to the Screen Actors Guild - Producers Pension and Health Plans?



Please visit the Contribution Forms section of the SAGPH website to find detailed
instructions on the verification process to be followed when reporting pension and
health contributions on behalf of choreographers. These instructions are pursuant
to section 6.G of Schedule J of the Codified Basic Agreement and Schedule A.I.F.F.3
of the Commercials contract.

It is important that eligibility verification procedures are followed so that the
Plans can accept and process the contributions without any unnecessary delay.
